Support Worker - CDSW-3000H

COMMUNITY DISABILITY SUPPORT WORKER 1 – (3000H)

Reports to:

Team Leader

Chief Executive Officer

Overall Responsibility:

By following policies and procedures and the individuals Annual Service Plan, the CDSW - 3000H will provide support, care and training to clients in their care.

Qualifications:

  • Must be a legal adult;
  • Must have completed Grade 12 or equivalent;
  • Be willing and able to successfully complete all training requirements outlined in the Employee Training policies.

Duties and Responsibilities

1. Clients:

  • Ensure that the rights of the client are upheld at all times;
  • Assist and support the client to ensure that basic needs are met;
  • Assist in the implementation of the client’s Annual Service Plan;
  • Assist in the development of Annual Service plans and other assessments / programs that are required.

2. Reporting:

  • Complete all necessary documentation as per the policies and procedures of this agency;
  • Complete data collection required for behaviour programs, planned approaches, restrictive practices;
  • Complete incident reports for non-typical situations.

3. Overall Responsibilities:

  • Be at work according to the working roster outlined by your Immediate Supervisor;
  • Inform those you report to of any concerns, problems or difficulties that prevent or hamper you from providing services to the client in the ways outlined in the Policy and Procedure manual;
  • Attend and participate in staff meetings and training sessions as required;
  • Assist in promoting positive public relations in the community;
  • Perform other duties deemed necessary by your Immediate Supervisor(s).
  • It is the responsibility of the CDSW 1-3000H employee to ensure that all scheduled hours are completed. If the employee cannot complete the hours that are scheduled, the Immediate Supervisor will attempt to fill the shift with another employee so the individual does not miss their scheduled outing.
  • Employees must be flexible with the times and activities that are planned. In the event that an employee can’t accommodate an individual’s chosen activity, another employee will be asked to accompany the individual.
  • If the individual should choose to cut an outing short, the CDSW 1-3000H employee will return the individual to their home. The scheduled hours will continue in home (if the individual requests. If the employee chooses to end their shift, they will be paid only until the time they left.
  • Employees will provide encouragement and guidance to individuals to attend new activities. Employees need to be aware of the activities that their community has to offer and will provide the individual choices of activities to attend.
  • Attend and participate in Annual Service plans, staff meetings and training sessions as required.
