The Community Disabilities Services Practitioner in the Residential Program is an experienced frontline role with moderate reporting/documentation duties, with the responsibility to support individuals with home living in their community. They are to ensure individuals are supported in being as independent as possible, maximizing their personal well-being and being fully engaged and valued members of their communities.
We are seeking a full-time Practitioner to work 30 hours per week who is self-motivated and a team player. The successful candidate must be willing to work variable shifts including nights and rotating weekends.
High School Diploma, valid driver’s license, reliable vehicle and clean Criminal Record check are required. Proof of vaccinations for COVID-19 (min. of two) will be part of a conditional offer of employment.
